Make a referral to GOTLEHS CARE

Send us the suburb, the supports and the hours. You will get a real answer on capacity, not a maybe.

Participants can refer themselves. So can families, support coordinators, plan managers, hospital discharge teams and allied health professionals. There is no minimum number of hours and no obligation to move all of a participant’s supports across at once.

What happens after you send it

We acknowledge the referral during business hours and tell you who is handling it.
We check capacity against the suburb, the support types and the shift pattern you have described.
We come back with a straight answer: when we could start, or that we cannot service it.
If it goes ahead, we arrange a conversation with the participant and put a service agreement in plain English.

Short-notice discharges

If a discharge date is close, ring rather than use the form. Call 0425 799 687 between 9am and 5pm Monday to Friday.
